There are no automatic excuses or exemptions with the exception of statutory exemption from jury service. Prospective jurors may be exempt, excused, or deferred from jury service in accordance with the Ohio Revised Code. Excuses can only be granted by the Judge assigned to this jury or his/her representative.

PLEASE NOTE: Any request to be excused from jury duty must be received by the Court no later than 10 business days before your reporting date. This form may be submitted electronically by clicking the Submit button below. However, if supporting documentation is required, you must mail or fax this form and all documents. (See contact information above.) If you have any questions, you may call the above number.

If you are asking to be excused for a medical reason, documentation from a doctor or other medical professional must be mailed or faxed with this request.
